In case of emergency card, plastic cover + Medic form

I just thought this may be a good idea as I have made one for myself.
It isn’t the lanyard type, this is credit card shape in a plastic wallet,

Each card contains a Medic form to contain all of your personal information in the event of an emergency or even when visiting the hospital. Ideal to be kept in pocket, purse or wallet.

At their simplest, medical alert cards carry key information about a person’s condition, needs or communication preferences. That might include a diagnosis, emergency contact details, medication information, or a short message explaining what support may be helpful.
